Children’s Community Nursing

The Community Children’s Nursing (CCN) service works together with GPs, paediatricians, hospitals, health visitors, school nurses, social workers and voluntary agencies to enable children with acute and chronic conditions to be cared for at home and school.

The team is made up of eight different services:

  • Continuing Care Team
  • Paediatric Virtual Ward
  • Paediatric Continence Team
  • Paediatric Diabetes Team
  • Paediatric Epilepsy Team
  • Paediatric Virtual Ward
  • School Educational Needs Team
  • Short Intervention Chronic Conditions

Paediatric Virtual Ward

Virtual ward aims to support earlier hospital discharges and prevent hospital admissions. We have different pathways that we can take children onto which are:

  • Asthma/viral induced wheeze
  • Bronchiolitis
  • Jaundice
  • Diarrhoea and Vomiting
  • Eczema
  • Disimpaction regime with constipation
  • Nephrotic Syndrome
  • Tonsillitis

What to do out of hours

Out of hours, if you are under the care of virtual ward currently please ring the Paediatric Assessment Unit on 0121 507 3266 or if you are concerned that your child is unwell attend A&E.
If you are concerned about your child and not currently under the virtual ward team please contact your own GP, 111, or attend A&E.

Paediatric Continence Team

Our aim is to promote healthy bladder and bowels for children aged four to 19, who live or go to school in the Sandwell area. We aim to increase children’s independence with toilet skills within their personal capability. The service can also provide teaching/information sessions on:

  • Delayed toileting
  • Constipation and soiling
  • Day and nighttime wetting
  • Continence products

What to do out of hours

Families need to contact either their GP, urgent care provider, NHS 111 or go to A&E if they have any concerns out of the service hours.

Epilepsy

The Roald Dahl Marvellous Children’s Charity provide specialist nurses and support for seriously ill children by working in partnership with the NHS. There are currently over 80 Roald Dahl Specialist Nurses caring for over 24,000 children across the UK.

Our clinical nurse specialists offer support, information and advice to families and liaise with other professionals. As clinical experts they can answer questions about your child’s condition, work closely with the consultants and local services, monitor and adjust treatments.

What to do out of hours

For urgent medical advice, contact your GP or NHS 111. For medical emergencies call 999.

Short Intervention Chronic Conditions

The short intervention chronic care team provide support to patients with long term, chronic conditions and their families. They provide training for families and schools, end of life care and support for palliative patients. Additionally, the team see seriously ill patients in the community to complete treatment after being discharged and they help to refer patients to healthcare professionals.

What to do out of hours

Patients under the CCN team will have open access to Sandwell Paediatric wards out of CCN hours. They need to call the ward before arriving.
