Community Rehabilitation Facilities

The Community Rehabilitation Service is made up of a team of specialists who work with anyone over 16 who needs rehabilitation. We will work with people and their families to improve their independence, whatever their diagnosis may be. We also work closely with Social Services.

Care can be provided in people’s homes, a day hospital, residential or nursing homes, and clinics.

Our team of nurses, therapists and support workers provide ongoing support for patients with:

  • all progressive neurological diseases – Parkinson’s disease, motor neurone disease, multiple sclerosis and Huntington’s disease
  • stroke
  • the elderly who are frail and need help to regain independence
  • falls
  • osteoporosis

We also provide the therapy input into palliative care (support for the end of life), intermediate care and STAR (Short Term Assessment and Reablement) service. This is a Social Services home care scheme which provides up to six weeks of therapy and reablement in a patient’s own home.

We host the Home Accident Prevention Service, providing minor changes to people’s living environment to make it safer (e.g. carbon monoxide testing).

Routine referrals are seen within two weeks, urgent referrals seen the same day. Patients can refer themselves by calling the number in their locality.

Community Rehabilitation on the wards and in the community

Therapists will speak to a patient who needs rehabilitation after they are discharged from hospital about how best to continue their treatment in their home or in a specialist rehabilitation healthcare facility.

Wherever possible, the team will arrange for their care to continue in their home environment, but where that isn’t possible, the person may be referred to our Specialist Rehabilitation Wards at Rowley Regis Hospital (Eliza Tinsley Ward and Henderson Ward) or Leasowes Intermediate Care Unit. They may also be referred to Harvest View, in Rowley Regis, which is a health and social care facility jointly run by Sandwell Council and the Trust.

This will give them tailored short-term care to re-learn the skills required to keep them safe and independent at home, or to adapt to be as independent as possible.

The aim is to help people regain their ability to complete usual activities, such as managing their own personal care, or becoming more mobile. The team at the wards will work with each individual on their own goals to help them achieve a level of function which will allow them to return home. It is possible that the team will arrange ongoing care and support, which may include further rehabilitation in their own home.

Each ward is nurse and therapy-led, and the medical cover is provided by a team of GPs and Advanced Clinical Practitioners. We also have visiting clinicians from a wide range of professions who will come to see individuals on a case-by-case basis.


Community Ward D43

The Community Ward D43 – also known as a Medically Fit for Discharge ward – is based at Sheldon Block, Ground Floor, City Health Campus.

We take patients who are deemed medically stable to be discharged but there may be a delay in going home. We focus entirely discharging them safely and efficiently with support from their family.

We have experts in discharges and the sometimes complicated process that comes with it. We have discharge liaison nurses, community case managers, social workers and an occupational therapist.

Nursing staff  are trained to recognise if you become unwell again.

The unit is nurse-led and there are GP visits three times a week, but there is also access to the out of hours service – speak to a member of the team for more information.

We encourage patients to wear their own clothes and to make use of the dining and lounge areas. Family participation is greatly encouraged, and we have an open visiting culture (between 11am-9pm).


Leasowes Intermediate Care Centre

Each patient being cared for at Leasowes has their own room with their own en-suite bathroom area.

There is a large therapy gym, assessment kitchen, as well as shared dining facilities to support social contact. There are indoor and outdoor spaces for those receiving care to meet with visitors away from their bedspace if they prefer.

You will need to bring day and night clothes and toiletries, and everyone is encouraged to used the shared facilities during the day including for dining as part of their ongoing rehabilitation. Whilst our team are on hand to help you, you will be encouraged to be as independent as possible and will be central in the decision making about your goals and progress.


Rowley Regis Hospital

Each ward at Rowley Regis has 24 beds, made up of single-sex bays of 6-8 beds as well as some individual side rooms. Bathroom facilities are shared.

There is access to a large therapy gym, assessment kitchen and shared dining facilities to support social contact. There are indoor and outdoor spaces for those receiving care to meet with visitors away from their bedspace if they prefer.

You will need to bring day and night clothes and toiletries, and everyone is encouraged to use the shared facilities during the day, including for dining, as part of their ongoing rehabilitation. Whilst our team are on hand to help you, you will be encouraged to be as independent as possible and will be central in the decision making about your goals and progress.


Harvest View

Harvest View in Rowley Regis offers specialist support from both social care and health staff all under one roof, with 80 en-suite rooms and friendly communal areas as well as lovely outdoor spaces.

The facility enables people to get back home after a hospital visit and also those who need some structured support to avoid a hospital stay altogether.

Harvest View replicates the home environment, where residents can be encouraged back to their normal health in familiar surroundings. The focus is on maintaining and improving independent living rather than specifically treating the medical condition.
